Power Of Attorney

A power of attorney is a legal document wherein a trusted person is appointed to take care of your financial and legal affairs on your behalf.  A power of attorney is the document used to stipulate who should manage your affairs when you're unable to, under what circumstances, and any specific wishes you want to be upheld. An attorney-in-fact is a person you name in a POA document who will help manage your affairs when the POA goes into effect.

A Durable Power of Attorney helps you plan for medical emergencies and declines in mental functioning and can ensure that your finances are taken care of. Having these documents in place helps eliminate confusion and uncertainty when family members have to make tough medical decisions. Health Surrogate

A health care surrogate designation is a legal document that appoints a person to become your “surrogate” if you become incapacitated.  Your health care surrogate is a person you authorize via a Designation of Health Care Surrogate form to make medical decisions for you when you are unable to make your own decisions. It is important that you discuss your wishes in advance with your health care surrogate.

 

Last Will  And Testament

A last will and testament is an estate-planning document that explains and clarifies what you want to happen with your property at the time of your death, among other things. Your will provides all the essential and necessary details of who will inherit your property.

Living Will

A Living Will is a declaration in which you state your wishes to have all or certain types of medical treatment withheld or withdrawn if you are suffering from a terminal and irreversible condition, including a profound comatose state with no reasonable chance of recovery.
